Iodine, as the substrate in thyroid hormone synthesis, plays key roles in regulating and maintaining normal thyroid function. Assessing the iodine status is necessary in evaluating thyroid function, and in clinical research on iodine-related nutritional balance . About 90% of the dietary iodine is excreted in the urine, and therefore, urinary iodine (UI) has been widely used for evaluating iodine nutritional status, especially when assessing recent iodine intake.

 However, even after correction for creatinine levels, UI is easily influenced by the amount of food and water consumed. Hence, the World Health Organization (WHO) only recommends UI in evaluating population iodine levels and not for individual iodine status.

 On the other hand, serum iodine (SI) is reportedly useful in diagnosing thyroid disease and is believed to be the most accurate and reliable biomarker for assessing bioavailable iodine to the thyroid gland, and it can provide more direct information than UI; furthermore, SI is not easily influenced by food and water consumption. Moreover, no free iodide can be found in the blood, as iodide is immediately trapped by the thyroid cells. Any iodide that is not trapped is secreted in the urine, resulting in over 80% of urinary iodine to be iodide.
